FREE CARE TO MANAGE DIABETES AND HYPERTENSION
Community Health Center Chronic Disease Management Clinic is exclusive to diabetes and hypertension (high blood pressure), and to aid our patients with any health issues they currently may be managing.
Even though many chronic diseases come with serious health risks, and can be lifelong, we educate patients to manage them through diet, medications, healthy lifestyle habits, exercise and a strong family support systems.
Our patients learn to control their health issues so they can live exciting, healthy, and fulfilling lives.
Our services are FREE to those who qualify.
Our Services
Our hypertension treatments include:
- High blood pressure management
- Stress management
- High blood pressure medications
- Diet changes and exercise
- *equipment (when available)
Our diabetes management includes:
- Constant monitoring of your blood-glucose levels
- Glucose lowering and diabetes medications
- Exercise and physical activity management plan
- Food intake record and scheduling
- *equipment (when available)

We Need Your Help
Our services rely on contributions from our community.

Please consider:
$6,000/Year – you will sponsor a critical, medically complicated patient (i.e.: heart failure), to have access to all our services (including ultrasounds, X-rays, MRI’s) and access to specialty services such as psychiatric, pulmonology, urology, and others.
