What To Bring
NEED TO HAVE YOUR IDENTIFICATION
On your first visit to Community Health Center of West Palm Beach, you will need to bring the following information for qualifying purposes.
Picture ID (at least one)
- Driver’s License
- Official State of Florida ID card
- Passport of origin country
- Any picture ID deemed acceptable by our staff
- Child must have a birth Certificate and must be accompanied by parent or legal guardian
Proof of Family Income (copies of any form of income for the current year from list below)
- W2 or 1099 Contract Work
- Payroll Check Stub (full-time or part-time work)
- Tax Form (IRS Form 1040)
- Unemployment compensation (if unemployed)
- Supplemental Social Security Income (SSI)
- Social Security Disability Income (if disabled)
- Or any other form of income (including letter of support by person giving shelter)
For those who are homeless, please provide a homeless declaration letter (police department Saint Ann’s, etc.)
Proof of Palm Beach County Residency (at least one)
- Driver’s License
- Electric or utilities bill from your current address
- Or any official document deemed acceptable by our staff
